Formstack Outage History

We've identified an issue affecting confirmation emails sent through Formstack Forms. The Confirmation emails from forms are currently delayed or not being delivered, including test emails sent from the settings screen. Notification emails remain unaffected.

We are currently investigating an issue where some customers may receive unexpected account deactivation notifications related to bounced confirmation emails. Our team is actively working to identify the cause. We will provide additional updates as more information becomes available.

Microsoft is currently experiencing an Exchange Online service issue that may impact customers using Microsoft 365 to send email through Formstack. Customers using Microsoft 365 to send merged documents through Formstack may experience merge errors and failed email deliveries. Customers may also encounter an error when attempting to authenticate or re-authenticate their Microsoft 365 email connection. No other Documents delivery services are impacted. Microsoft is working to restore service. We will provide additional updates as they become available.

We have identified an issue impacting the migration of large forms. While our team investigates, please refrain from migrating large forms to avoid potential issues or failed migrations. We are currently investigating reports of users receiving errors when accessing Formstack Documents through the OnlyOffice builder experience. At this time, we are seeing impact specifically related to the OnlyOffice builder. Native builder functionality, Manage Files access, and PDF uploads are currently operating as expected. We are investigating reports that some users are unable to save changes to existing forms, publish form updates, or duplicate forms within Formstack Forms for Salesforce. Our team is actively reviewing the issue and gathering information to determine the scope and root cause. Based on our initial investigation, the issue appears to be related to Salesforce Summer '26; however, we are continuing to validate this finding and work toward a resolution. Due to recent Salesforce OAuth/security changes, some customers may experience Salesforce Delivery/authentication failures. Affected users may experience merge or delivery failures accompanied by errors such as: “INVALID_SESSION_ID: Session expired or invalid” “Could not refresh your Salesforce access token” In some cases, re-authenticating the Salesforce connection temporarily restores functionality, though reports indicate the issue may recur.Our Engineering team is actively investigating. We will continue to provide updates as we learn more.

We are investigating reports of customers being intermittently prompted to sign in while signing Formstack Sign documents.

We are currently investigating an issue where the Formstack Sign API may not reflect updated document statuses after the signing process is completed. Signing workflows are completing successfully, but some integrations may not receive the correct status updates. Our team is actively working to identify and resolve the issue.

We are currently investigating reports of errors affecting some Streamline projects. Impacted users may see a “Something went wrong” message while progressing through affected workflows. Our engineering teams have identified a likely frontend issue introduced in a recent deployment and are actively working to confirm root cause and prepare remediation. We will provide another update as soon as more information is available.

We are investigating an issue affecting workflow sessions in Streamline (us.streamline.intellistack.app), where users may be unable to run sessions in production. Workaround: As a temporary solution, replace intellistack.app with formstack.app in Session URLs.
