Is Formstack down?

We are investigating an issue causing authentication errors and preventing some users from accessing Formstack Documents after signing in.We’ll provide an update as soon as more information is available.

Access to Formstack Documents has been restored, and we are monitoring to confirm continued stability. Document merges were not impacted and continue to complete as expected.

This incident has been resolved.

We’ve identified an issue affecting Streamline workflows that include an email (SMTP) step without a file attachment. Sessions that reach one of these email steps may fail instead of sending the email, and the session doesn’t complete. We’re working on a fix and will share an update when available.

Customers of Spectrum using Spectrum's Security Shield may experience being blocked when trying to execute a new workflow. We have confirmed with Spectrum that they have removed us from their block list but the block may remain in effect for the next 24 hours for some subset of users

Spectrum has removed us from its Security Shield block list. Some Spectrum customers may still be blocked from starting a new workflow while the change takes effect. Spectrum advised that this may take up to 24 hours for some users.

We're aware of the critical PHP vulnerabilities disclosed by the PHP project on October 21, 2026. Like much of the web, some of our services use PHP, and our Engineering and Security teams began working on this as soon as the advisory was announced. Here's where things stand: - We're preparing to apply the security updates and have put additional protective measures in place in the meantime. - We're actively monitoring our environment, and to date we haven't identified any evidence that these vulnerabilities have been exploited or that customer data has been accessed. - No action is required on your part. - We don't expect service disruption. Streamline: Update - October 21, 2026, 2:23 PM ET We've completed applying the PHP security updates to Intellistack Streamline as of 2:23 PM ET. Work on the other affected services is underway, and we'll post an update here as each is completed.  Our Engineering and Security teams are continuing to monitor our environment closely.  To date, we haven't identified any evidence that these vulnerabilities were exploited or that customer data was accessed. No action is required on your part.

Formstack Forms: Update - October 21, 2026, 7:15 PM ET We've completed security updates for Formstack Forms across both our US and AU environments. The updates were applied and validated as of 7:15 PM ET on October 21. Streamline and the Formsite public website were updated earlier today, and work on Formstack Documents and Formstack Admin is underway. Our monitoring still shows no evidence that these vulnerabilities were exploited or that customer data was accessed. No action is required on your part. We identified an issue that caused some Formstack Sign documents to be finalized and delivered without an audit trail. The issue affected documents on October 8 from 7:42 PM to 9:31 PM ET. The signing process has recovered, and newly completed documents are processing normally. We are implementing a recovery script to restore the missing audit trails for affected documents. As part of this recovery process, some customers may receive a duplicate notification that their document has been finalized. If you previously received this notification, no action is required, and the duplicate message can be disregarded. We apologize for the inconvenience.
