Clinicians are currently unable to submit Add Activity documentation. On submit, the note returns to "Continue Draft" and does not appear in the patient's History. Draft content is being retained, so no documentation has been lost — notes can be re-submitted once this is resolved. This affects all patients. Our engineering team is actively investigating. We will post the next update within the hour.
