We are currently investigating an issue where Auto-Recaps are not being posted for all meetings where it's enabled. Some completed meetings are not generating or posting their expected Auto-Recaps. Issue with loading or opening Decisions in Microsoft Teams

We are experiencing an issue affecting our Microsoft Teams app “Decisions”, starting today at approximately 2:00 AM IST. Our Teams app is no longer loading inside Microsoft Teams and displays the following error: “There was a problem reaching this app”. This is a widespread issue with Microsoft and several Teams apps have the same experience. Use app.boarddecisions.com as alternative to MS Teams

We’ve implemented a workaround for an upstream Microsoft Teams issue. The Decisions app is now loading normally in Teams again. If you’re still seeing “There was a problem reaching this app,” please restart Teams and try again (or use app.boarddecisions.com). We’re continuing to monitor.

We are currently experiencing intermittent availability issues with Decisions AI. This is due to an ongoing service incident in Microsoft Azure OpenAI Service (Sweden Central), which Decisions AI depends on. Microsoft has reported elevated error rates and reduced availability in the region and is actively working on mitigation and recovery. We have discovered a partial issue with authenticating to the Decisions application, leading to interrupted service for some users. The issue has been identified and is cause by a loss of service from Microsoft Network Infrastructure. We are working with Microsoft to restore these services as soon as possible. We are investigating a service degradation for creating Meeting Recap with AI. The functionality still works, but can tak up to multiple minutes to finalize. Based on insight up to this point, it seems to be related to Azure OpenAI server processing performance.

We are continuing to see reduced performance and cases of failures with Meeting Recap. We are working with Microsoft to improve the service for the Azure OpenAI environment.

The Meeting Recap process is still experiencing service degradation. The number of failures have been drastically reduced, but some users will experience slower than normal processing of Meeting Recap. The service degradation is caused by performance issues in the MS Azure OpenAI infrastructure. We are actively working with MS product teams to improve the performance.

Meeting slides are having issues merging content to PowerPoint with the latest release of Microsfot 365 apps. Versions earlier than 2410 works just fine. Currently affecting customers on the "Current Channel" running the latest update, as that is the only one on 2410 so far: https://learn.microsoft.com/en-us/officeupdates/update-history-microsoft365-apps-by-date. Issue has been identified and a fix is being prepared for release

Regional outage in Australia

On Monday morning, we received reports from few customers in the Australia region indicating that the Decisions platform was returning a 503 Service Unavailable error. Upon investigation, we identified that one of our globally distributed app services, specifically located in the Australia region, was experiencing the 503 error on random basis. We promptly raised a support ticket with Microsoft, and a service restart successfully resolved the issue. We are continuing our investigation with Microsoft to understand the root cause.

- Problem: The impact occurred intermittently starting in April/May 2024 and was resolved in June 2024. The information about connected Planner Plans to a given Teams Team Channel were in some instances incorrectly cleared from the Decisions API and the reference object, ultimately leading to displaying the Plan as removed or deleted in the Decisions app, although everything was still there. - Root Cause: Upon investigating all possible paths that could have cleared the information programmatically, we have identified a couple of places in the code were the logic were incorrectly implemented. In some error conditions, these incorrect implementations could lead to the clearing of channel reference information when not intended. - Solution: All identified cased that could have lead to incorrectly removing Channel reference have been identified, resolved and fix released
