    On April 20, 2026 at 02:00 PM PDT, Autodesk identified an issue where customers were unable to create reports containing photos in Forma - Reports across the Canada, Germany, India, UK, Japan regions. Once the issue was resolved on April 21, 2026 at 10:28 AM PDT, Forma - Reports returned to normal operation. This is a retroactive notification as this issue did not display on the Autodesk Health Dashboard at that time. Thank you for your patience and understanding.

    **AUTODESK EVENT ANALYSIS** **Incident Number:** #COE-INC141611 **Incident Date:** April 20, 2026 ‌ **Summary** On April 20, 2026, between 2:00pm PDT and April 21, 2026, 10:28am PDT Autodesk Forma Build experienced a service disruption affecting Report creation containing Photos. ‌ **Impacted Services** * Report generation containing Photos in the UK, Canada, India, Germany and Japan regions. ‌ **Root Cause** * An incorrect Infrastructure deployment resulted in the Reports service encountering errors during generation of Reports for entities that contained Photos. * The incorrect configuration was corrected and redeployed, resolving the issue. ‌ **Autodesk Actions** Autodesk has completed a post-incident analysis of the event and identified actions to be taken. These include the following: * Updated internal deployment documentation to standardize the infrastructure deployment process. * Identified and addressed gaps in deployment validation procedures. * Improvements to monitoring and validation of configuration-related deployment issues. ‌ Thank you for your patience and understanding.
