    On May 19, 2026, at 5:11 AM PDT, Autodesk identified an issue where a subset of customers had missing events related to user login and user attachment views in FPTR. Once the issue was resolved on May 19, 2026, at 6:28 AM PDT, FPTR returned to normal operation. This is a retroactive notification as this issue did not display on the Autodesk Health Dashboard at that time. Thank you for your patience and understanding.

    **AUTODESK EVENT ANALYSIS** **Incident Number:** # coe-inc143776-shotgun **Incident Date:** May 19, 2026 ‌ **Summary** On May 19, 2026, between 05:14 AM PDT and 06:34 AM PDT, Autodesk experienced an issue that prevented a small set of event logs to not be created on a small number of customer Flow Production Sites. Most of the missing events \(>90%\) were attachment view events, with a smaller subset of login related events. ‌ **Impacted Services** * Flow Production Tracking ‌ **Root Cause** * A code deployment introduced an incompatibility with the underlying database, causing a subset of event logs to fail to be created for a small number of customer sites. The issue was resolved by rolling back the release. ‌ **Autodesk Actions** Autodesk has completed a post-incident analysis of the event and identified the following actions: * Improving internal deployment procedures to ensure database migrations and code changes are validated together before release * Introducing automated checks to flag incomplete migrations prior to deployment * Requiring database migrations to be reviewed and merged independently of application code changes ‌ Thank you for your patience and understanding.
