   **AUTODESK EVENT ANALYSIS** **Incident Number:** #coe-inc154127-acsforms-p **Incident Date:** September 03, 2026 ‌ **Summary** On September 03, 2026, between 02:36 AM PDT and 08:39 AM PDT, some Autodesk customers in the EU region experienced issues loading Forma Build projects containing newly generated activities. A backend service responsible for retrieving project activity data encountered errors, preventing affected projects from loading correctly. ‌ **Impacted Services** * Forma Build ‌ **Root Cause** * The incident was caused by an incorrect deployment configuration in the EU region, which resulted in a version being used before its required database update had been applied. * Autodesk resolved the incident by correcting the deployment configuration and restoring the affected service to the intended production version. ‌ **Autodesk Actions** Autodesk has completed a post-incident analysis of the event and identified actions to be taken. These include the following: * Auditing and correcting production deployment configurations across all regions to ensure the intended versions are in use. * Additional validation to ensure production environments use approved versions and their required dependencies. * Monitoring improvements for the affected APIs to enable faster detection and response. * Improvements to the deployment platform and release process. ‌ Thank you for your patience and understanding.
