The US SL3 swimlane experienced service disruption issues on Jan 25th between 03:30AM PST - 04:30AM PST. Clients on the US SL3 Swimlane, may have experienced system slowness and intermittent error (504 Gateway Error) while accessing the application during the impacted period. Cornerstone engineering team addressed the issue as a top priority and confirmed the issue to be resolved. We will publish the details on the root cause for this incident within 7-10 business days.

**Incident Summary:** On January 25th, 2026, clients hosted in the US SL3 Production environment experienced temporary portal access issues. **Impact:** During the incident window, users encountered timeout errors, which resulted in intermittent inability to access the portal. **Root Cause Analysis \(RCA\):** The issue occurred following a planned maintenance activity. After the maintenance, an internal service dependency did not fully recover, which temporarily disrupted communication between system components and led to access timeouts. **Resolution:** The environment was restored to its prior stable state, which resolved the underlying communication issue. Portal access was confirmed to be fully restored following this action. **Preventive Measures:** To help prevent similar incidents in the future, Cornerstone is taking the following steps: * Strengthening post-maintenance validation checks to ensure all services recover as expected * Enhancing monitoring to detect service communication issues more quickly
