    **Incident Summary:** On July 15, 2025, users in the US AWS Production environment intermittently encountered “404” errors when attempting to access the Manage Candidate feature within the application. This resulted in temporary disruptions for a subset of users. **Root Cause:** The issue was caused by a temporary misconfiguration in the request routing system. A caching issue prevented the latest code deployment from being applied to all incoming traffic, leading to intermittent failures and 404 errors on the Manage Candidate endpoint. **Corrective Action:** The engineering team identified that the most recent routing update was not fully in effect due to the caching behavior. The affected service was restarted, and traffic was redirected to the correct version, restoring full access to the Manage Candidate feature. **Preventive Measures:** To prevent similar incidents in the future, Cornerstone has taken the following actions: * Enhanced monitoring and alerting for routing logic and feature availability to enable faster detection and resolution of similar issues in the US Production environment * Deployed the fix across all impacted regions and environments to ensure consistent performance and long-term stability
