  1. investigating Sep 30, 2026, 04:52 PM UTC

We are currently observing increased processing times for requests to the OpenAI model text-embedding-3-large in the US region. Requests may consequently take longer than usual. Our team is investigating the cause. We will provide an update as soon as more information is available.

  2. monitoring Sep 30, 2026, 06:05 PM UTC

We are currently seeing improvements and increased stability across affected environments following the earlier disruption impacting the OpenAI model. Processing times have returned toward normal levels, and most requests are now processing with normal processing time. We are continuing to closely monitor system performance.

  1. resolved Aug 20, 2026, 09:48 AM UTC

On 11 August 2026, from 14:30 to 19:50 CEST, a subset of virtual agent instances experienced degraded performance on conversations processed via the OpenAI (via boost.ai) vendor configuration, caused by elevated latency on the upstream provider's models. During this window, affected conversations experienced extended processing times and a elevated number of timeouts or errors. Provider latency returned to normal at 19:50 CEST, after which all affected instances were stable. Total duration of degraded performance: 5 hours 20 minutes. Note: this incident entry was published retroactively on the 20th of August as part of our commitment to a complete and accurate public service record.

  1. identified Jun 08, 2026, 10:00 AM UTC

We are currently observing elevated latency affecting a subset of requests to Azure OpenAI models hosted in Europe. Users may experience intermittent timeouts or increased response times in LLM-powered features. Our team is actively investigating the issue and has engaged Microsoft Azure support to assist in identifying the root cause and restoring normal service as quickly as possible. We will continue to provide updates as more information becomes available.

  2. monitoring Jun 08, 2026, 10:41 AM UTC

We are currently seeing improvements and increased stability across affected environments following the earlier disruption impacting Azure OpenAI services. Error rates and latency have returned toward normal levels, and most requests are now processing successfully. We are continuing to closely monitor system performance and working with Microsoft Azure to ensure full resolution and sustained stability.

  3. resolved Jun 08, 2026, 03:28 PM UTC

This incident has been resolved. Microsoft has confirmed that the issue affecting elevated latency in requests routed to the models has been fully mitigated. Our systems are stable, and we are no longer observing elevated latency. We will continue to monitor performance to ensure ongoing reliability.

  1. investigating Apr 15, 2026, 10:18 PM UTC

We are currently observing an elevated error rate affecting a subset of requests to an Azure OpenAI model. Specifically, some requests are failing when routed to the model responsible for out-of-scope detection and few-shot . Users may experience intermittent failures or unexpected errors in AI-powered features that depend on this functionality. The root cause has been identified as an issue on the Microsoft Azure OpenAI side. We are actively working with Microsoft to expedite resolution and restore normal service. We will provide updates as more information becomes available.

  2. monitoring Apr 16, 2026, 04:23 AM UTC

Microsoft has implemented a fix for the issue causing elevated error rates in requests routed to the out-of-scope detection and few-shot models. Our systems have recovered, and we are seeing error rates return to normal levels. We are continuing to closely monitor performance to ensure stability. We will provide a final update once we confirm the issue is fully resolved.

  3. resolved Apr 16, 2026, 08:10 AM UTC

This incident has been resolved. Microsoft has confirmed that the issue affecting elevated error rates in requests routed to both the few-shot and out-of-scope detection models has been fully mitigated. Our systems are stable, and we are no longer observing elevated error rates. We will continue to monitor performance to ensure ongoing reliability.
