  3. investigating Oct 08, 2026, 12:46 PM UTC

Summary: On 30th September 2026, Aera identified a platform availability issue impacting the Production East US environment through automated monitoring. The Production Services Engineering (PSE) team promptly initiated an investigation. The issue was identified as an underlying storage infrastructure condition that temporarily impacted database operations and affected service availability. Remedial actions were undertaken to restore the affected database service, following which services were successfully restored. Subsequent validation and monitoring checks confirmed that the affected environment had been restored to full operation. Customer Impact: During the outage window, affected customers experienced temporary unavailability of the Aera platform within the Production East US environment due to the disruption to the underlying database service. Root Cause: The issue was caused by an underlying storage infrastructure condition that temporarily impacted database operations and prevented normal processing from completing. This subsequently affected service availability. Remediations: Aera’s engineering teams investigated the underlying infrastructure condition and performed the necessary recovery actions to restore the affected database service. Once the database service was restored, the impacted services recovered successfully. Validation and monitoring checks were carried out to confirm full recovery and normal platform operations. Future Mitigating Actions: Aera is reviewing its operational response processes to enable faster identification of similar conditions and more timely initiation of recovery actions, helping to reduce overall recovery time and customer impact. Existing monitoring and alerting capabilities will also be reviewed for opportunities to further support early detection and response. In parallel, Aera is working with our infrastructure provider to understand the factors that contributed to the recovery time and will incorporate any additional recommendations identified through their analysis.
