# Overview On 14 January, we identified an issue affecting the creation of USD Virtual Accounts. While existing accounts remained accessible, some newly created accounts were not immediately available as expected. The issue was resolved the same day, and normal service has been restored. # Root Cause The incident was caused by a failed internal database migration related to compliance settings. A missing permission prevented part of the system from completing updates required for Virtual Account creation, resulting in API errors and missed account creation events. There was no direct service outage or data loss. Some customers may have experienced difficulty retrieving newly created Virtual Accounts. Affected customers were advised to use an alternative endpoint to retrieve account details while the issue was being resolved. # Remediation and Prevention We resolved the issue by restoring the required database permissions and performing a controlled restart of the affected services. This allowed pending Virtual Account creation events to be processed successfully. We’re strengthening safeguards, improving recovery processes, and enhancing testing to reduce the likelihood of similar issues in the future. ###### Currencycloud Confidential
