    During the investigation, the system operations team discovered that Assessment Builder was unable to establish a connection to the database systems. As a result, assessment authoring and delivery were impacted for approximately 1 hour and 15 minutes. No impact was observed on the Administration, Customer Management, or Talent applications. This issue appears to have been within the internal systems and the issue has been resolved.

    Our investigation revealed that at approxiamately 2025-09-23T12:29:10Z \(UTC\) we experienced a temporary service disruption caused by an unexpected and extreme surge in network traffic. At peak, our systems were servicing over 20,000 simultaneous requests per second, which impacted Application Builder performance for a significant segment of our customers. Initial analysis of our Application Firewall \(AF\) logs shows that requests were valid and there is no indication of malicious activity. Our System Operations team reviewed and refreshed affected systems to restore system services performance and availability. We will continue to investigate the root cause data and apply additional operational enhancements aimed at improving the platform’s ability to handle unusual activity of this nature.

    We're currently investigating multiple reports of errors occurring during Assessment Builder Certificate generation. Our development team is working to identify the root cause.

    Our development team has identified an issue with the default US Letter-sized certificate background design that may prevent certificates from displaying properly for some customers. This issue does not affect the A4 sized certificate background or any alternate certificate designs that are used, such as the Brillium modern or alternative designs (available at: https://support.brillium.com/en-us/knowledgebase/article/KA-01027/en-us). We are actively working on a resolution and will continue monitoring the system in the meantime.

    **ISSUE** After a security update, monitoring services reported that some customers may be experiencing intermittent 503 errors. This issue impacted a small number of customers. **ROOT CAUSE ANALYSIS** Investigation by our DevOps team identified the root cause as a configuration issue that affected an underlying systems application web service. **RESOLUTION** A revised web service configuration was deployed to resolve the issue. Further monitoring indicated that the service was functioning normally and as expected. **NEXT STEPS** The DevOps team is conducting further analysis to prevent the issue from resulting in any future service degradation.
