A partially failed SFP module in our switch infrastructure caused this outage. The SFT module was redundant but it did not classify itself as being failed (it was still processing packets) but the fail rate was high. The failure rate was enough to cause a failover due to the failover timeout threshold. Manually shutting down the SFP restored normal operations via the redundant connection and a new SFP will replace the failed one.
